WHAT IS YOUR NEW PATIENT PROCEDURE?
Please follow the link https://app2.luminello.com/kamlehshabanmd/form and click "request consultation." Once you've filled out the form, Dr. Shaban will approve the request. From there, you can sign up for an appointment online through her electronic health record.
Your first appointment will be a 90min new evaluation. During this time, she will review your symptoms, history, medications tried before, family history, social history, etc. Then, you will be provided a working diagnosis, some education around your illness, and a treatment plan. Your plan may involve medications, therapy, nutrition, movement, and lifestyle changes.
WHAT AGE GROUPS CAN BE TREATED?
All age groups. Dr. Shaban is a board certified physician in child, adolescent, and adult psychiatry
WHAT CONDITIONS ARE TREATED?
Dr. Shaban provides treatment for depressive disorders, mood disorders, anxiety disorders, PTSD, bipolar disorder, OCD, and ADHD, to name a few.
WHAT SERVICES ARE PROVIDED?
Currently we offer psychiatric medication management with Dr. Shaban. Some patients may choose to engage in therapy with Dr. Shaban as well.
IS INSURANCE ACCEPTED?
At this time, Dr. Shaban is accepting insurance through a third party called Headway! We accept Cigna, Aetna, and Anthem now, with plans to expand! If you're looking to use your insurance follow this link!
ARE SUPERBILLS AVAILABLE?
Yes, Dr. Shaban can provide you with a Superbill to turn into insurance. If you work with your insurance company you should be able to get reimbursed for part of the cost
DO YOU OFFER PAYMENT PLANS?
Yes! We provide a variety of individualized, interest free payment plans for patients based off their financial needs.
